
Geriatric Outreach Program
The North East Specialized Geriatric Centre (NESGC) offers a Geriatric Outreach Program in Hearst through the Nord-Aski Family Health Team to help older adults with complex health needs stay healthy, independent, and living safely in their community for as long as possible.
We provide specialized geriatric care and support for older adults and their caregivers, working together to understand your health concerns and create a plan that meets your needs.
How We Can Help You
Our team looks at the whole picture of your health — including medical conditions, mobility, memory, medications, mood, and daily activities. Our goal is to help reduce or slow down changes that may affect your independence and quality of life.
How to Access the Program
To access the program, you need a referral from a physician or nurse practitioner.
You May Be Referred If You:
Are 65 years of age or older;
Live in the community, including a retirement home;
Have complex medical concerns.
This Program Is Not for You If:
You are currently receiving care in a hospital or long-term care home.

What to Expect
If your referral is accepted, you will attend at least two appointments:

Your first visit will be with a geriatric assessor, who will spend time getting to know you and completing a detailed assessment

Your second visit will be with a geriatric physician, who will review the assessment and discuss recommendations with you.
During your visits, the geriatrics team will help put the recommendations into place and follow up on your progress. When possible, we work with your family doctor or nurse practitioner to support your ongoing care.
