Home Support Services

The Nord-Aski Family Health Team is an approved service provider for Ontario Health atHome (OHaH), delivering care directly to patients in their homes in our community.

 

Through this partnership, eligible patients may receive Personal Support Worker (PSW) services in their home, based on assessed needs. These services may include assistance with daily living activities, personal care, and support that helps patients remain safely and comfortably at home.

 

Services are coordinated through Ontario Health atHome, with referrals and care planning completed in collaboration to ensure coordinated, patient-centred care.
